Documents required for registration:
- A copy of the resident’s ID or Individual Civil Status Record, plus a copy of the Family Civil Status Record issued within the last two months.
- A copy of the guardian’s/responsible person’s ID, Family Civil Status Record, home address, and phone number.
- A copy of the insurance card (if available).
- A letter of reference from a parish or official authority (such as a Mukhtar or Mayor), in case any assistance is required.
- A complete medical file for the resident, including:
- A recent medical report (issued within one week) from the treating physician, clearly outlining the resident’s physical and psychological health condition.
- A recent medical prescription (issued within one week) listing all current medications.
- Previous laboratory test results or imaging reports related to the resident.
- Results of any medical tests required by the nursing home prior to admission.
